I have gone through several "phases" of note taking. Each has had the same basic requirement - it has to be ink on paper, and it has to fit in my shirt pocket. My current choice is 3x5 cards carried in a leather holder. Of the several versions of that system, I have settled on the Shirt Pocket Briefcase from Levenger. It doesn't look like I will be trying anything else for a while - this one works for me. Kind of "old school", but then I'm the guy who still carries a slide rule around.
The "Pocket Briefcase" isn't cheap, but before I got it, I tried the cheap ones from Office Depot; they aren't worth it. Funny how that "you get what you pay for" adage seems to be true most of the time.

My grandfather was a WWII vet and machinist. He rarely went anywhere w/out his slide rule and small notebook.
PS - I like the pocket briefcase concept.

Sorry, just now seeing this response. I was referring to the pocket brief case. I will see if I can find it and if I do, you can have it.
